After you have filled out a registration form for yourself, you can fill out the form below to add your children (under 12).

  • Copy of Passport
  • Name on Passport
  • Passport #
  • Passport Expiration Date
  • Date of Birth
  • Country of Issuance
  • Citizenship

This must be received by our offices in order for your registration to be fully processed. Our office fax is 1-408-927-9830. If you have any questions, you may contact us at 1-408-927-9052. If you don't have a fax machine, you can often use one for a small fee at your nearest copy shop. Responsibility: Harvest Evangelism and their agents, offices and/or suppliers of services pursuant to or in conjunction with the itinerary in this brochure, shall act only as agents for the passenger in making arrangements for hotels, transportation, restaurants or any other service and do not assume any liability whatsoever for any injury, damage, death, loss, accident or delay to a person or property due to an act of negligence of or default of any hotel, carrier, restaurant, company or person rendering any of the services included in this brochure or by any act of God. Further, no responsibilities are accepted for any damage or delay due to sickness, pilferage, labor disputes, machinery breakdown, quarantines, government restraints, weather or other causes beyond their personal control. No responsibility is accepted for any additional expense, omission, delays, rerouting or act of any government authority. The airlines concerned are not to be held responsible for any act, omission, or event during the time passengers are not on board their planes or conveyance. The passage contract in use by the airlines concerned when issued shall constitute the sole contract between the airlines and purchaser of this package and/or passenger. The right is reserved to withdraw any portion of the package and/or make such changes in the program as may be found desirable for the convenience of the parties and the proper carrying out of the tour package. The price of this package is based on rates in effect (including foreign currency exchange rates) at the time of printing and are subject to change at any time without notice. Participants are responsible for contacting appropriate embassies to assure that any visa requirements are met before the trip. Harvest Evangelism does not assume responsibility for any errors or omissions in the content of this brochure.
